Our Mission
Partnerships Adult Day Center is committed to enabling its participants to remain living at home, supporting the quality of life for participants, family members and caregivers.
Partnerships accomplishes this mission by providing a medical model adult day care center serving frail elders, and mentally or physically impaired adults in the greater New Haven area.
What is Adult Day Care?
Adult day care, as a service, lies somewhere between a senior community center and an assisted living facility. It’s a solution for seniors who want to stay in their homes, but need a certain level of medical attention each day – someone to count their pills correctly, or check them for signs of worsening conditions. Our patients might not need constant supervision, but at this stage of life, daily check-ins can be crucial. And for primary caregivers, adult day care helps them just as much as it helps the patients themselves. What do you do with an aging parent, or even a spouse, who’s starting to develop either physical or mental health or cognitive function challenges, while you are running your own life with a family and/or work responsibilities? Bring them to Partnerships.
Our Staff
Partnerships’ caring staff includes an Executive Director, Registered Nurses, Certified Nursing Assistants, and a Recreational Therapist.
A variety of volunteers participate in regular and special programs, providing additional support and companionship.
We Are Certified
Partnerships Center for Adult Day Care, Inc. d/b/a Partnerships Adult Day Center, is certified by the Connecticut Association of Adult Day Care, Inc. as a Medical Model Adult Day Care Center.
